## What it calculates

- eGFR by the race-free CKD-EPI 2021 creatinine equation, for CKD staging.
- The same eGFR de-indexed to absolute mL/min via Du Bois body surface area, which is the value KDIGO 2024 and the FDA 2024 renal-impairment guidance call for when dosing at extremes of body size.
- Creatinine clearance by Cockcroft-Gault, which is what most drug-dosing tables are still written against, on actual weight and on the ideal/adjusted dosing weight.
- The KDIGO G-category, the albuminuria A-category, and the risk category where the two meet.

## Inputs

- Serum creatinine (mg/dL or µmol/L)
- Age
- Sex
- Weight and height (Cockcroft-Gault, body surface area, dosing weight)
- Urine albumin-to-creatinine ratio, optional (mg/g or mg/mmol)

## Caveats

- Both equations assume steady-state creatinine. They are not valid in acute kidney injury.
- CKD-EPI 2021 is validated in adults aged 18 and over; under-18s need CKiD U25 or bedside Schwartz.
- CKD-EPI 2021 deliberately omits a race coefficient.
- A G-category is not a CKD diagnosis on its own — KDIGO requires more than three months of persistence and stages by cause, GFR and albuminuria together.
- The output is an estimate for clinical judgement, not a substitute for it.
